This site republishes official records, aggregated for readability. Nothing here is opinion, user reviews, or paid placement.

Sources

  • Provider Information — CMS Provider Data Catalog (ratings, staffing, chain affiliation)
  • Health Deficiencies — every inspection citation since 2017, with severity
  • Ownership — CMS ownership & control filings (owners, officers, percentages)
  • Penalties — federal fines and payment denials

All four refresh monthly from data.cms.gov; the current build uses the July 2026 release. "Ownership group" pages reflect CMS's own chain-affiliation field — we do not guess affiliations.

Severity codes (A–L)

Each deficiency carries a scope/severity letter. A–C: no resident harm. D–F: potential for harm. G–I: actual harm. J–L: immediate jeopardy — the most serious category, meaning resident health or safety was in immediate danger.

Corrections

Every fact traces to a source file. If you believe a record is wrong, email data@carerecords.org — we check against the official file and correct within 5 business days. Facilities cannot pay to alter or remove records; the only path to changing a record here is changing it with CMS or the state.
