Florida Assisted Living: Facility Records & How to Check Violations

3,014 licensed facilities · 1,433 with sanctions or fines on record · 1079 with a Class I citation (Florida's most severe).

Florida licenses about 3,000 assisted living facilities through the Agency for Health Care Administration (AHCA). Florida has the most usable public records of any state:

  1. Go to FloridaHealthFinder.gov (the state's official portal).
  2. Use Facility/Provider search → Assisted Living Facility and search by name, city, or county.
  3. Each facility profile shows complaints, sanctions, and deficiency data, plus bed count and occupancy.
  4. Inspection report PDFs are public under Florida Statute 429.19 — request them from the profile page.

What to look for: repeated medication-management deficiencies, elopement (resident wandering) incidents, and any sanction with a fine — these are the patterns Florida regulators cite most.

Questions to ask on a tour, records in hand

  • "Your last inspection cited X — what changed since?" (bring the citation date)
  • "What is your direct-care staff turnover?" (high turnover is the strongest quality signal in research)
  • "Who owns this facility, and how many others do they operate?"
