All four refresh monthly from data.cms.gov; the current build uses the August 2026 release. "Ownership group" pages reflect CMS's own chain-affiliation field — we do not guess affiliations.
Each deficiency carries a scope/severity letter. A–C: no resident harm. D–F: potential for harm. G–I: actual harm. J–L: immediate jeopardy — the most serious category, meaning resident health or safety was in immediate danger.

Facilities cannot pay to alter or remove records; we correct our own transcription, linkage and display errors directly.
